Prevention Education Notice (Grades PreK-12) and Teen General Wellness Screening Notice (Grades 6-12)
All Melissa ISD students, in accordance with state legislation, are required to have parental consent on file with the school in order to participate in prevention education (Grades PreK-12) and the Teens Can Survive general wellness screening program (Grades 6-12 only) during the 2024-2025 school year. Each of the notices linked below include detailed information about each of these requirements and a link to a parental consent form that must be completed by 5 p.m. on Friday, Sept. 20, 2024.

Library Book Checkout (Grades 6-12)
The notice linked below includes detailed information about the parental consent requirements and options regarding your student’s permission to check out books from their school library. No action is required for students in Pre-K or for students in grades K-5. For students in grades 6-12, parental consent is required in order for the student to check out books from the school library that are outside of the standard juvenile collection. Please review the notice for more information and a link to the consent form for your student’s school.
